Wet gangrene

This occurs naturally in moist tissues and organs eg. mouth, bowel,lungs, cervix, vulva.

CAUSES :

  • Diabetic Foot is  example of wet gangrene due to high sugar content in the necrosed tissue which favours growth of bacteria.
  • Due to pressure on sites like sacrum ,buttocks, and heels   Bed Sores occur in bed ridden patient
  • Develops rapidly due to  blockade of venous and less commonly arterial blood flow from THROMBOSIS  or EMBOLISM.

IDENTIFICATION: The external surface of the small intestine is identified by absence of taenia coli band and appendices epiploicae

DESCRIPTION:  The affected part is suffused with blood causing hemmorhagic infarction which may be due to thrombo-embolic occlusion of the superior mesentric artery commonly.in arterial occlusion the line of demarcation between the gangrenous bowel and the normal bowel  is usually sharp while venous occlusion may lead to a more diffuse appearance.

GROSSLY:

it is a cut and mount formalin fixed specimen of small intestine.

The affected part is dilated.

External surface there is swelling and dark tan coloration in the affected area,  may be dark purple ,green, and black.(1)

Internal surface (mucosa) shows hemmorhage and necrosis.Luminal surface shows blunted and tan mucosal folds in the affected segment (2) while the unaffected area shows gray and sharp mucosal folds. The two are separated by a line of demarcation (3)
